Thank you for your interest in the Boise Cooperative Preschool. Enrolling your child is the beginning of a rewarding experience for both you and your child.

Please read the BCP Member Responsibilities (see Member Responsibilities), which describes the obligations of being a member of BCP. All forms must be completed and returned with the appropriate fees to complete your child’s registration. Only after you have submitted all fees, required forms, and documents will your child be accepted for enrollment – dependent upon space availability and on a first-come basis at (or after) the Open House held in February. Please allow 3-4 weeks for a response from the BCP registrar regarding your child’s acceptance.

List of registration materials to be returned:

• Signed Boise Cooperative Preschool Membership Application, which includes the By-Laws statement, Permission to Publish, Hold Harmless, and Student Safety Statement to be initialed by the applicant.
• Boise Cooperative Preschool - Health & Student Information form
• Emergency Medical Treatment of Children consent form
  
• Idaho State Police Bureau of Criminal Identification Name Based Criminal Background Check Form
• Boise Cooperative Preschool – Emergency Information
• Copy of Immunization Records or completed immunization exemption form.
• Non-refundable Registration Fee & Background Check Fee (see Registration & Fees) made payable to Boise Cooperative Preschool – must be paid only by check or money order (no cash)
